
Non-clearing vitreous hemorrhage is a common surgical indication in patients with diabetic retinopathy and other retinal vascular diseases. In this case, a patient with a dense vitreous hemorrhage secondary to peripheral exudative hemorrhagic chorioretinopathy (PEHCR), we discuss some surgical considerations when performing vitrectomy. These include pre-operative assessment with b-scan ultrasonography, assessing the lens status, safe positioning of the cutter mouth when retinal landmarks cannot be visualized, removing the vitreous hemorrhage around the cutter trocar site, and being judicious with the extent of peripheral shaving.
