85. To operate or not to operate?

This patient has a history of advanced diabetic retinopathy with poor vision in both eyes.  This left eye has undergone TRD repair at an outside hospital and has significant residual emulsified oil and a chronic full-thickness macular hole.   Would you operate on this eye?  What clinical and imaging data would you use to guide your decision-making?  These are the questions facing first year UCLA vitreoretinal fellow Dr. Prashant Tailor.  Let’s see how he and his attending manage this patient and at the end we will look closer at the OCT findings that can help guide you when you are faced with this question.  Thank you Dr. Tailor for sharing this case!
