
How would you remove this lens? This patient has a mature lens that has dislocated to the posterior pole. You have a number of options including removal with the vitreous cutter, mobilizing the entire lens into the anterior chamber and removing it through a scleral tunnel, or emulsifying the lens in the posterior segment using the fragmatome.
