Sometimes, even with a careful scleral depressed examination in the clinic, the causative retinal break in rhegmatogenous retinal detachments cannot be identified. Serous or exudative retinal detachments need to be ruled out, but if the surgeon is convinced that the detachment is rhegmatogenous in nature, sometimes intraoperative identification of breaks may be necessary. In today’s case, presented by guest surgeon Dr. Ahmed Javed from Lincolnshire, England, intraoperative scleral depression failed to reveal the break. In this scenario, Dr. Javed employs the dye extrusion technique to identify an occult retinal break. It’s an elegant approach and at the end of the case we will discuss some tips for finding occult retinal breaks.
