There are many techniques for secondary lens implantation in patients without capsular support for a posterior chamber or sulcus IOL. One intraocular lens that is specifically designed for this scenario is the Carlevale IOL, which is a sutureless scleral-fixated IOL. The Carlevale lens is familiar to European vitreoretinal surgeons but may be less familiar to surgeons in other parts of the world. In today’s video, guest surgeon Dr. Lamberto La Franca, a vitreoretinal fellow in Brussels, Belgium, shares a beautiful case of scleral fixation of a Carlevale IOL in a patient with a subluxated in-the-bag single-piece IOL. This case was performed entirely by Dr. La Franca under the supervision of his mentor, Dr. Ernesto Bali.
