154. Staged Diabetic TRD Repair

In cases of complex vitreoretinal pathology, sometimes multiple surgeries are necessary and a staged approach may be preferred to safely return to the operating room when the eye is stabilized.  In today’s case, presented by returning guest surgeon Dr. Jesus Maticorena from Peru, the patient underwent a pars plana vitrectomy to repair a complex diabetic tractional retinal detachment.  Due to the presence of significant subretinal fluid, Dr. Maticorena performed limited segmentation and laser during the first surgery with a plan to return to the operating room once the subretinal fluid had resorbed.  This video picks up at the second surgery, six weeks after the primary vitrectomy. Remarkably the retina was completely attached allowing Dr. Maticorena to safely complete the surgical repair.  At the end of the video we discuss some of the pros and cons of this surgical approach.
