Posterior hyaloid elevation is a critical step when performing vitrectomy for a rhegmatogenous retinal detachment. In diabetic patients this step can be challenging due to the presence of vitreous schisis, tight vitreoretinal adhesions and the lack of counter-traction when elevating membranes and vitreous from detached retina. Our guest surgeon is Dr. Ramon Bringe Freitas, a vitreoretinal specialist at Evangelico Vila Velha Hospital in Brazil. His patient is a 42 year old phakic diabetic male with mild tractional membranes who now presents with a macula-involving rhegmatogenous retinal detachment. Let’s check out the surgical approach and thank you Dr. Bringe Freitas for sharing this case!
