One of the most important first steps before starting a vitrectomy is to visually confirm the placement of the infusion line trocar in the vitreous cavity. But what do you do when there is a significant media opacity that blocks your view? This is exactly the dilemma presented In today’s case by senior UCLA vitreoretinal fellow Dr. Adrian Au. The patient has advanced proliferative diabetic retinopathy previously status post panretinal photocoagulation at an outside hospital and presents with a very dense vitreous hemorrhage and baseline vision of light perception. Dr. Au has brought the patient to the OR to perform a combined lensectomy and vitrectomy but can’t see the infusion trocar.
