Subretinal fluid drainage during scleral buckling is a step that can be complicated by either retinal incarceration or subretinal hemorrhage. When the macula is detached, subretinal bleeding can settle under the macula and compromise post-operative outcomes. This is the issue presented in todays’ case by returning guest Dr. Frederic Aissani. The patient is a teenage girl with a chronic macula-involving retinal detachment with subretinal bands. There is no PVD and Dr. Aissani has decided to repair the detachment with a scleral buckle. All is going well until subretinal fluid drainage is performed. Check out what happens and how Dr. Aissani managed the case. At the end we will discuss steps you can take to prevent subretinal hemorrhage and some additional management options. Thank you Dr. Aissani for sharing this case!
