
Submacular hemorrhage can be a complication of a variety of conditions including neovascular AMD, polypoidal choroidal vasculopathy and retinal arterial macroaneurysms. When the hemorrhage is thin, often this can be managed with intravitreal anti-VEGF injections alone. However for thicker hemorrhages, displacement may be considered since thick hemorrhages may interfere with RPE outer-retinal homeostasis, cause oxidative or toxic damage to the retina and/or contract and mechanically damage photoreceptors. In today’s case, our guest surgeon of the week, Dr. Omar Shakir, demonstrates surgical displacement using submacular tPA and 12% C3F8 gas implantation. At the end of the case we discuss some risks and surgical considerations when performing this procedure.
