109. ERM peeling with the vitreous cutter

There are many ways to peel visually significant epiretinal membranes from the macular surface.  Most commonly this is done with intraocular forceps using a pinch and peel technique.  Alternatively, a flex loop or a Tano scraper can be used to fracture the ERM and these instruments or intraocular forceps can be used to peel the ERM.  Another option is to propagate the ERM peeling using the vitreous cutter.  That is the technique employed by guest surgeon, Dr. Omar Shakir, who is sharing with us a week of surgeries performed in an office-based surgery center.  This patient has a 20/30 erm associated w

ith metamorphopsia.  The ERM was peeled at first with forceps but Dr. Shakir found that the ERM kept fracturing and was coming off in strips.  Using the assistance of Tissue Blue stain, he completes the case by engaging an edge of the ERM with the cutter and allowing him to remove the ERM entirely.  At the end of the case we discuss some visualization tips with ERM peeling as well as the pros/cons of concurrent ILM peeling.
