Today’s case comes from returning guest surgeon Dr. Ramon Bringe Freitas from Vila Velha, Brazil, at Evangelico Vila Velha Hospital. His patient is a 48-year-old diabetic patient presenting with light perception vision, a dense vitreous hemorrhage, and a tractional retinal detachment. The patient underwent phacoemulsification with lens implantation and anti-VEGF injection one week before vitrectomy. We discuss the management of complex proliferative diabetic retinopathy, pearls on surgical visualization, timing of intervention, and intra-operative decision-making. Thank you, Dr. Bringe Freitas, for sharing this case!
